Three Rivers launches community health dashboard

Three Rivers Public Health Department has launched a new online Community Health Assessment and Improvement Plan (CHA/CHIP) Dashboard, providing residents and community partners with access to local health data for Dodge, Saunders, and Washington counties.

The tool, developed on the My Sidewalk platform, transforms complex health data into an accessible, interactive format. It allows users to explore key indicators, including population demographics, access to care, health behaviors, chronic disease, cancer screening rates, and leading causes of death.

This innovative tool brings community health data to life, making it accessible, interactive, and meaningful for everyone in Dodge, Saunders, and Washington Counties. The dashboard is also available directly on the Three Rivers Public Health Department website, where users can click on “2026 Community Health Assessment Data” to explore local data and better understand factors that impact community well-being.

“We wanted more than a Community Health Assessment that simply checks a box,” said Terra Uhing, executive director of Three Rivers Public Health Department. “I wanted something meaningful, something our communities could truly use. This tool provides accessible, up-to-date data that empowers our residents, partners, and leaders to better understand our health and take action. This is exactly what we were hoping to create.”

The dashboard represents a significant step forward in how data is shared and used across the region. Health outcomes are influenced by more than clinical care and are shaped by factors such as environment, access to resources, and daily behaviors. By providing clear, local data, the dashboard helps communities better understand challenges, identify strengths, and take action.

The platform is designed to:

· Support data-driven decision-making

· Increase transparency and public awareness

· Align efforts to improve health outcomes

The dashboard highlights a wide range of key indicators, including community conditions, preventive health behaviors, healthcare access, chronic disease and cancer screening rates, mortality data, and Community Health Improvement Plan priorities.

By making this information widely available, the Three Rivers Public Health Department aims to strengthen partnerships and support ongoing efforts to improve health outcomes across the three-county service area.

The dashboard is intended for use by residents, healthcare providers, community organizations, and policymakers as a shared resource to guide planning, education, and community action.
