
Three Rivers Public Health Department is inviting residents of Dodge, Saunders, and Washington Counties to participate in the 2026 Community Health Assessment (CHA).
The Community Health Assessment is conducted every three to five years and helps identify key health needs, concerns, and priorities in the communities we serve. The information gathered guides planning, partnerships, and future health initiatives across the region.
“If you live, work, or play in any of our three counties, we want to hear from you,” said Terra Uhing, Executive Director of Three Rivers Public Health Department. “This is your chance to give input and help us better understand what is happening in the communities we serve.”
The survey questions were written to be easy to understand and can be completed in five minutes or less. The survey is confidential and open to all residents.
