Fremont police officers conducted a welfare check at about 10:30 Monday night in the 700 block of West 11th Street.
The welfare check turned into an assault investigation. That investigation resulted in the arrest of Jacob L. Vacha, 27, of Fremont for domestic violence assault-third/cause bodily injury.
