Three Rivers Health Department Conducting Needs Assessment Survey

FREMONT – The local health department needs your input as they work to identify the needs and concerns of the tri-county area.

Three Rivers Public Health Department, Saunders Medical Center, Methodist Fremont Health, and Memorial Community Hospital & Health System are collaborating to  conduct a Community Health Survey. Everyone who lives, works, or attends school in Dodge, Saunders, or Washington Counties are invited to take this survey. “There’s a lot of different questions about health related things,” on the questionnaire, according to Lacey Taylor from Three Rivers.

The survey will be open from June 22nd to July 19th.  “Everyone’s survey input is important to us and provides us with information about the health of our communities that is not available from other sources,” said Cork. “Your input will help us to identify key health needs throughout our three-county service area and will help guide our efforts and plans in the future.”

The survey link will be posted on the Three Rivers Public Health Department Facebook page, or you can click here to go directly to the questionnaire. Hard copy surveys will also be available at the Three Rivers Public Health Department at 2400 N Lincoln Ave, Fremont, NE 68025.

The health department is hoping to receive at least 500 responses to the survey, which will take 20-30 minutes to complete. As a thank you for completing the survey, participants can enter to win one of six $100 grocery gift cards. Two winners will be awarded in each county of the survey area.
